BRAVES BASEBALL AT PARKVIEW FIELD
The Braves baseball team will play at Parkview Field this week. On Tuesday, May 7 at 7pm, the Braves will play Snider. Tickets will be $7 and available through TinCaps.com and ParkviewField.com or at the gate on gameday.

2023-24 SPORTS REGISTRATION ON FINAL FORMS
- Athletes are not eligible to practice with their teams if they are not listed as ELIGIBLE on Final Forms.
- Once you have completed all forms and have uploaded the Sports Physical, your student should turn green and be listed as ELIGIBLE on Final Forms.
Blackhawk Christian Athletics is proud to announce that we have partnered with FinalForms to streamline our compliance paperwork. FinalForms is an online registration tool that will take the place of our traditional vital forms packet. This is the packet that includes state mandated paperwork as well as school required items like emergency contacts and insurance information that must to be filled out annually. One of the greatest benefits of FinalForms is that coaches and athletic trainers will have instant access to emergency information via their mobile device. Also, once information is entered, you will only have to update it each year. This will save a lot of time!
If you have a student-athlete who will be participating in athletics at Blackhawk Christian during the 2023-2024 school year you MUST create an account with FinalForms.
The IHSAA physical form is below and needs to be completed by a doctor and uploaded (Pages 1-3) to the Final Forms site by the parents for each student.
