Sports Physicals

ALL students in grades 6-12 planning to participate in a sport during the next school year must have an updated sports physical dated April 1, 2022 or after in order to tryout or practice with their team. You can access the required paperwork at the links below. Please schedule your appointments for your sports physicals to be completed. Once completed, turn in the hard copy sports physical form into the high school office. All athletes are encouraged to visit their family physician for their annual physical. The other forms below can be completed and signed online. ALL FORMS MUST BE COMPLETED IN ORDER FOR THE ATHLETES TO ATTEND PRACTICES.
