I, the parent or legal guardian do hereby give permission for the below mentioned person to participate in said camp hosted by BCS. I acknowledge that I will not seek to have Blackhawk Baptist Church Inc. d/b/a Blackhawk Ministries held liable in the event that any accident, injury, loss of property or any other circumstance or incident occurs during or as a result of my child’s participation in the camp. This release of liability includes accident, injury, loss, or damages to my child, as well as, to other individuals or property which may result from my child’s participation in the camp, even if it is the result of negligence by the school or one of its employees. I hereby release and agree to hold harmless the ministry, school, and employees, from any claims arising out of my child’s participation in the camp.
I understand that, in the event medical treatment is required, every effort will be made to contact me. However, if I cannot be reached, I give consent to receive treatment and/or hospital care to be rendered to the above mentioned minor under the supervision of the BCS coaching staff and on the advice of any physician or surgeon licensed to practice medicine.
The parent/guardian gives permission for BCS to photograph the player for the purpose of promotion of the camp or team. All photos will be available upon request.
